Why Modern BBL Surgeons Stay Above the Muscle: The Anatomy Behind the Single Most Important Safety Rule
The shift from intramuscular to subcutaneous-only fat grafting is the biggest technical change in Brazilian Butt Lift surgery in a decade. Here is the plumbing that explains it.
If you read only one technical detail before consulting about a Brazilian Butt Lift, make it this one: where the fat goes matters more than how much fat goes in. Over the past several years, the field has moved decisively toward subcutaneous-only grafting, meaning fat is placed in the layer above the gluteal muscle and its covering fascia, never inside the muscle itself. Understanding why requires a short tour of buttock anatomy, and it explains most of the safety questions patients ask.
The gluteus maximus is not just a muscle. It is a highly vascular structure threaded with the superior and inferior gluteal veins, large low-pressure vessels that drain directly toward the heart. When liquid fat is injected into or beneath the muscle, two failure modes become possible. First, a cannula tip can lacerate one of these veins directly. Second, and more insidiously, fat injected under pressure into a tight muscular compartment can be pushed into a torn vein even without a direct hit, because the pressure inside a swollen muscle exceeds the pressure inside the vein. Fat that enters the venous system travels to the lungs. This is a pulmonary fat embolism, and it is the mechanism behind nearly every BBL fatality studied in autopsy reviews.
The evidence for this is unusually clear by plastic surgery standards. A multisociety task force review published in 2018 examined deaths associated with gluteal fat grafting and found a consistent pattern: fat within or deep to the muscle, injured gluteal veins, and fat in the pulmonary vessels. Critically, the reviewed deaths involved intramuscular or submuscular placement. Cases where fat stayed in the subcutaneous layer did not show this pattern. The anatomic conclusion was straightforward. The gluteal fascia, the fibrous sheet covering the muscle, acts as a barrier. Keep the cannula above it, and the large veins are effectively out of reach.
This finding changed technique in several concrete ways. Surgeons following current guidance use rigid cannulas of larger diameter, typically around 4 to 5 millimeters, because thin flexible cannulas can bend and dive deeper than intended. Injection is performed while the cannula is moving, not while it is stationary, so no single spot receives a high-pressure bolus. The cannula is angled parallel to or away from the muscle, never steeply downward. And increasingly, surgeons use intraoperative ultrasound to watch the cannula tip in real time and confirm it remains above the fascia. Florida made ultrasound guidance a regulatory requirement for gluteal fat grafting in office surgery settings in 2022, and several professional societies now describe it as the emerging standard.
A reasonable question follows: if fat can only go in the subcutaneous layer, does that limit results? Somewhat, and honestly, yes. The old rationale for intramuscular injection was that muscle is well supplied with blood, so grafted fat might survive at higher rates there. That theoretical benefit never justified the risk, and it turns out the subcutaneous space is more capable than assumed. The layer above the fascia in the buttock is relatively thick and expandable, and skilled surgeons can distribute meaningful volume through it in multiple passes and planes within that layer. What subcutaneous-only grafting does change is the ceiling on single-session volume. A patient seeking a very dramatic change may need a staged approach, with a second session after six months or more, rather than one aggressive session. Most clinicians would call that a feature, not a bug, since oversized single-session grafts also suffer from poor survival: fat that is packed too densely outstrips its blood supply and dies, sometimes forming oil cysts or firm nodules.
For patients, this anatomy lesson translates into practical screening questions. Ask a prospective surgeon directly: Do you inject only into the subcutaneous space? Ask whether they use ultrasound guidance to confirm cannula depth, what cannula size they use, and whether they follow the published gluteal fat grafting safety recommendations. A surgeon current with the literature will answer these without hesitation and will not be offended by the questions. Vague answers, or claims that intramuscular injection is fine in experienced hands, should be treated as a red flag, because the mortality data did not spare experienced hands.
One last clarification, since terminology gets muddled online. Subcutaneous placement is sometimes marketed as a skinny BBL or a safer BBL, as if it were a boutique variant. It is not a variant. It is the technique itself, as currently recommended. The distinction worth paying attention to is not branding but verification: whether the surgeon can confirm, ideally with imaging, that every pass of the cannula stayed on the safe side of the fascia. The procedure's risk profile improved substantially once the field understood this single anatomic boundary. The patients who benefit most are the ones who know to ask about it.
